Nurse Practitioner, Not for Profit – Long Term Care

The St. Demetrius (Ukrainian Catholic) Development Corporation is a not-for-profit charitable organization providing compassionate, culturally-sensitive care, home and services to seniors. We are comprised of a 152 bed long-term care home in the heart of Etobicoke and a 259 unit independent seniors apartment. We also offer a wide range of community programs and services to seniors living in the community.

The resident and family, as core members of the interdisciplinary care team, are the primary contributors to the residents’ individualized Plan of Care. Our care and services philosophy is demonstrated through our integrated programs of care modalities, therapies and services that address the total well-being of each resident. Position Overview:
The Attending Nurse Practitioner (NP) collaborates with the Director of Care and Medical Director to ensure the long-term care home’s (LTCH) policies, procedures and protocols for medical services. The Attending NP is a primary care provider to residents and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario.
Responsibilities:
- Collaborates with the resident and family/care giver and the health care team in the development, implementation and evaluation of each resident’s plan of care
- Guides and supports Director of Care, and nursing and personal care staff in challenging clinical situations
- Provides leadership and mentorship to long-term care home staff that enhances their knowledge, assessment skills and ability to care for residents in place
- Leads and collaborates in research, education and evidence-based practice initiatives to optimize the resident, long-term care home and health system outcomes
- Provides comprehensive primary care to residents as part of the interdisciplinary health care team
- Provides formal and informal teaching and coaching in clinical care, acting as a resource to interdisciplinary team members
- Performs a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews)
- Participates in internal and external committees
Qualifications:
- Current registration with the College of Nurses of Ontario as a registered nurse in the extended class (primary/health care of adult) and entitled to practice
- Post graduate education and experience in gerontology and/or palliative care (an asset)
- Experience in acute care or long term care (an asset)
- Demonstrates knowledge of legislation and standards and adheres to all legislation required
