Director of Care, Registered Nurse (RN) Long Term Care (NEW BUILD!)

Come join Partners Community Health (PCH) as the Director of Care will support the planning, transition and clinical operations at the new  new Long-Term Care home, with a combined 632 beds, which is set to open in south Mississauga in 2023.

Position Overview:

The Director of Care will report to the Executive Director. Along with the Executive Director, the Director of Care will operationalize the Model of Care in alignment with the Philosophy of Care for the new site and oversee the safe transition of the current residents.

Responsibilities:

  • Encourage positive relationships with all the residents, staff, and the community through active engagement with the Councils and Physician team.
  • Advances excellence in practice, ensuring effective CQI framework for each program approach.
  • Leads the direct care teams; including IPAC management, ADOCs, and Scheduling; while being adaptable and innovative on the floors to coach and encourage problem-solving and working to scope for each discipline.
  • Creates a resident living environment that is comfortable and safe and provide privacy, dignity, individuality and safeguarding of personal choice; liaises with family members as necessary
  • Promotes the Resident’s Bill of Rights and demonstrates sensitivity in all aspects of a resident’s life.
  • Ensures an interdisciplinary approach to care planning and implementation and supports the family’s/resident’s right to participate and be informed of the care delivered.
  • Monitors and assesses the medication and treatment routines including the provision of skin and wound care to ensure best practice and optimum support for the residents.
  • Leads medication management and ED transfer quality working groups.
  • Leads the quarterly Professional Advisory Committee, participates as a member of the Physician monthly meetings and meets weekly with the Medical Director; all to ensure safe, effective and excellent quality care.
  • Responsible for the implementation and maintenance of all standards and guidelines established by the Fixing Long-Term Care Homes Act and CMI requirements, Ministry of Health and Long-Term Care, the College of Nurses of Ontario and other applicable legislation.
  • In partnership with the ED and an interdisciplinary team, implements and oversees all system capacity initiatives to increase flow and facilitate safe transfers.
  • With the assistance of Human Resources recruits, hires, trains, and evaluates performance of personal care staff in accordance with the Employee Standards Act and any relevant collective bargaining documents while ensuring a learning culture.
  • Responsible to ensure employees work safely in compliance with accepted scope and safe work practice.
  • Will oversee and manage the transition of operations to the new home.

Qualifications:

  • Registered Nurse or Nurse Practitioner with a current certificate of competence from the College of Nurses of Ontario.
  • Current membership with the Gerontological Nursing Association (GNA) and Registered Nurses’ Association of Ontario (RNAO) is preferred.
  • Post-RN education, certification or degree in nursing administration and management, or business degree in health management, particularly in long-term care and further education in quality, gerontology, geriatrics and/or rehabilitation.
  • 5 years relevant management experience in a unionized long-term care facility preferred.
  • Fluency in both official languages and ability to speak in at least one other major tongue would be a definite asset.
