Session 1: Foundations of PSW Leadership in LTC

This session introduces PSWs to the fundamentals of leadership in LTC. Participants will explore their role as frontline leaders and learn practical strategies for improving care quality and team collaboration. Key topics include:
- The role of PSWs as leaders in LTC
- Communication techniques: active listening, conflict resolution, and effective reporting
- Managing workflows and supporting resident-centered care plans
- Policies and practices for ensuring resident safety and well-being
- Building trust and collaboration within care teams
Workplace Team Discussion:*l
After the live session, participants will lead a facilitated discussion with their home care teams to apply course concepts to their unique workplace environment. Activities include:
Reflection and Sharing: Each PSW shares a key takeaway from the session and discusses its relevance to their work with residents.
- Resident-Focused Planning: The team works collaboratively to apply leadership principles to specific resident cases, identifying ways to improve care and teamwork.
- Action Planning: The team sets practical goals for implementing leadership practices and improving care processes, with input from peers and supervisors.
Session 2: Leadership Applications for PSWs in LTC

Building on foundational skills, this session focuses on practical applications for PSWs to lead change and improve care quality. Participants will explore strategies for managing time, advocating for residents, and leading care team initiatives. Key topics include:
- Person- and family-centered care practices
- Leading resident care huddles and interdisciplinary meetings
- Managing change and resistance in LTC settings
- The 4Ps for high-risk residents: Pain, Positioning, Potty, Personal Needs
- Promoting teamwork, boosting morale, and preventing burnout
Workplace Team Discussion:
Back at their workplace, PSWs will collaborate with their home teams to discuss and implement strategies introduced in the live session. Activities include:
- Scenario-Based Practice: The team addresses real-life challenges, such as managing high-risk residents or responding to family concerns, using leadership techniques learned in the course.
- Team Building Exercises: Foster trust and improve communication among staff members to strengthen collaboration.
- Feedback and Goal Setting: Team members provide feedback on leadership efforts and set goals for sustained improvement in care delivery.
Practicum Workplace Project: PSW Leadership in Action: Resident Care Improvement
Objective:
PSWs will apply the leadership skills gained in the course to implement a resident-focused care improvement project within their LTC homes. The project promotes quality care, collaboration, and leadership growth among team members.
Project Components:
- Assessment:
Identify a specific area for improvement in resident care or team collaboration, focusing on high-risk situations or gaps in current practices. - Leadership Plan:
Create a detailed action plan with goals, timelines, and strategies for engaging the home team in addressing the identified issue. - Implementation:
Lead the initiative within the home team, facilitating care huddles and discussions to ensure team-wide participation and alignment with resident needs. - Evaluation:
Gather feedback from residents, families, and team members to assess the project’s impact on care quality and collaboration. - Presentation:
Present the project outcomes to supervisors and peers, highlighting successes, challenges, and opportunities for ongoing improvements.
This practicum project ensures PSWs bring the leadership concepts learned in the course directly into their workplace, creating meaningful and lasting improvements in resident care and team dynamics.
