Position Overview & Responsibilities:

The Clinical Director, Professional Practice & Integrated Care provides senior system-level leadership for professional practice, clinical governance, quality, education, and integrated models of care across Nucleus’ home and community-based services. The role will ensure the delivery of safe, high-quality, evidence-informed, and person-centered care aligned with Ontario Health priorities.

The Clinical Director is a key architect of Nucleus’ innovative models inclusive of the Florence Nightingale nursing initiative, and One TEAM interprofessional approach. This role bridges frontline clinical practice with organizational strategy, digital innovation, system partnerships, and workforce sustainability, enabling clients to remain safely at home while reducing system pressures.

Responsibilities:

Professional Practice & Clinical Governance

  • Provide organizational leadership for professional practice standards, scopes of practice, policies, procedures, and clinical protocols across PSW, nursing, and interprofessional teams.
  • Ensure nursing practice is optimized to support early intervention, escalation, and continuity of care in the community.
  • Embed professional practice standards and outcome measurement within nursing-led models of care.
  • Ensure compliance with regulatory college standards, legislation, professional accountability frameworks, and ethical practice.
  • Lead clinical risk management, incident review, critical incident analysis, and corrective action planning.
  • Maintain active relationships with regulatory bodies, professional associations, and sector partners.

Hospital-to-Home & Integrated Care Leadership

  • Lead the ongoing clinical design, implementation, and evaluation of Nucleus’ Hospital-to-Home model, supporting safe and timely transitions from hospital to community.
  • Advance integrated care pathways in partnership with hospitals, Ontario Health Teams, primary care, and community organizations.
  • Support community stabilization, ALC avoidance, chronic disease management, dementia, and complex care models.

One TEAM Interprofessional Model

  • Champion the One TEAM model, fostering strong collaboration among PSWs, nurses, care coordinators, and leaders.
  • Promote shared accountability, role clarity, and psychological safety across interprofessional teams.
  • Support leadership development and frontline engagement to strengthen team-based care delivery.

Quality, Safety & Data-Driven Improvement

  • Establish and monitor clinical quality and safety indicators, including falls, medication incidents, infections, ED visits, and hospital readmissions.
  • Leverage digital tools, data, and analytics to drive continuous quality improvement and proactive clinical decision-making.
  • Collaborate with Informatics (MITO) and Operations teams to strengthen data quality and performance reporting.

Education & Learning Organization Leadership

  • Develop and implement a corporate clinical education and learning strategy aligned with organizational priorities and emerging system needs.
  • Oversee onboarding pathways, competency frameworks, and continuing professional development programs.
  • Promote adult learning principles, evidence-informed education, and knowledge translation across the workforce.
  • Support student placements, preceptorships, and academic partnerships.

Infection Prevention & Control (IPAC)

  • Provide organizational leadership for IPAC policies, practices, education, and compliance.
  • Lead proactive IPAC risk mitigation strategies and vaccination programs.
  • Ensure alignment of IPAC practices with hospitals and system partners.

People Leadership, Innovation & System Partnerships

  • Provide leadership, coaching, and performance management to direct reports and emerging clinical leaders.
  • Collaborate with People & Culture on workforce planning, role design, and scope optimization.
  • Contribute clinical expertise to funding proposals, expressions of interest, pilots, and strategic partnerships.
  • Represent Nucleus in system, sector, and community forums.

Qualifications:

  • Regulated health professional in good standing (Registered Nurse preferred).
  • Master’s degree in Nursing, Health, Education, or related field.
  • Minimum 7–10 years of progressive clinical leadership experience, preferably in home and community care or integrated health systems.
  • Demonstrated expertise in professional practice, quality, education, and integrated models of care.
  • Strong understanding of Ontario Health priorities, Hospital-to-Home models, and interprofessional practice.
  • Formal education or demonstrated expertise in adult learning principles.
  • Exceptional leadership, communication, systems thinking, and change management skills with the ability to execute on initiative, projects, and models for excellent team and client/caregiver outcomes.

Compensation:

  • The position offers a salary range of $125,000 to $130,000 annually, commensurate with experience and qualifications, along with four (4) weeks of vacation.
  • Employees are eligible to participate in the HOOPP pension plan, and a comprehensive benefits package is provided through Blue Cross.
