Behavioural Nurse Coordinator, Registered Practical Nurse (RPN), Not for Profit – Long Term Care

About our Client:
Our organization is a non-profit charitable organization located in Unionville providing services to seniors since 1967. We, along with our affiliate corporations operate a long-term care home, rental housing for seniors, a seniors active living centre, and a life-lease apartment building. In addition, the organization provides community support services including an adult day program and geriatric assessment services. Our beautiful campus of care is home to over 350 residents and our vision is to be a progressive community where older adult thrive; aging well and living better.
Position Overview:
Reporting to the Director of Care, the Behavioural Nurse Coordinator (BNC) is responsible for the overall assessment and creation of interventions related to the behavioural support needs of residents and for leading the behavioural support program. The BNC provides direct clinical support to older adults presenting with or at risk for responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions residing in the long-term care home.
Responsibilities:
- Delivering resident-centred care, supported by evidence-informed clinical best practices, which recognize both the uniqueness of each resident (i.e., personhood) and an awareness of one’s own contribution to that relationship, including personal attitudes, values and actions.
- Assess, treat and safely manage the responsive/personal expressions associated with dementia and cognitive impairment
- Collaborating with the resident, their family and interprofessional team members to create, share, implement and model an individualized behavioural care plan.
- Demonstrate clinical reasoning and critical thinking skills that target prevention of the expressed responsive behaviours by creatively adjusting the social and physical environment; focusing on the resident’s abilities and knowing the individual, their life story and aspirations.
- Utilizing verbal and written communication strategies that demonstrate compassion, validate emotions, support dignity, and promote understanding about the individual as appropriate; including production of timely and accurate progress reports.
- Analyzing and evaluating the ongoing effectiveness of the implemented Plan of Care including thorough communication of next steps, suggestions for adherence, required adaptations and on-going follow-up.
- Providing facilitation, coaching, mentoring and demonstrating team leadership and change management skills with long-term care front-line staff.
- Participating in regular data collection as per the organization and provincial BSO regional reporting requirements.
Qualifications:
- RPN certification with the college of Nurses of Ontario in good standing
- Minimum 2 years experience working in a Long-Term Care facility
- Minimum 2 years experience in assessing and treating older adults with dementia, complex mental health, substance use and or other neurological conditions.
- Current certification in Psychiatric/Mental Health nursing or other relevant Geriatric Mental Health or Gerontology Certifications/Certificates considered a strong asset.
- GPA Coach, Montessori, PIECES, based programming trained is an asset or willing to take training.
- Must be available for flexible hours and alternate weekends.
- Proficient in computer and technological application.
